Change your name on the JP register
If you have recently changed your name, you should update your details with the Justices of the Peace Branch.
To use this form you must be a Justice of the Peace or a Commissioner for Declarations.
You must update your details with the Justices of the Peace Branch within 30 days of changing your name. This time limit is set by Section 22 of the Justices of the Peace and Commissioners for Declarations Act 1991.
What you will need
If you have recently changed your name by:
- marriage—you will need to attach a copy of your marriage certificate
- separation or divorce—you will need to attach a copy of either your marriage certificate or your birth certificate
- deed poll or through the Registry of Births, Deaths and Marriages—you will need to attach a copy of the deed poll documents or change of name certificate from the Registry of Births, Deaths and Marriages.
What happens next
Once you submit this form it will be automatically received by us. We will update the Justices of the Peace Register and contact you within 1 week to confirm your change of name.
Attachments must be less than 5MB in total.
